How Do I Enroll My Student at Wilson Middle School?

NEW TO AUGUSTA COUNTY?

STEP ONE: Complete New Family Online Registration by Clicking this Link (and click on the New Family Online Registration Green Box)

STEP TWO: After you have completed online registration, the parent or guardian must bring the following documents to the school.

  1. PROOF OF RESIDENCY: Per the School Board of Augusta County, you must submit ONE of the following documents to verify residence; they must be dated within the last 60 days, and only originals are accepted:

    • Lease Agreement or rent receipt with address indicated and signed by both parties

    • Mortgage contract or payment receipt with address indicated

    AND

  2. ONE of the following documents:

    • Utility bill (water, gas, electric, cable, or landline phone)

    • Personal property tax bill or receipt

    • Pay stub with address indicated

    • Bank statement with address indicated

NOTES: A driver’s license or state-issued ID does NOT serve as valid proof of residency. If you reside with someone else, you will be required to complete the Proof of Augusta County Residency (Notarized A & B) and submit a copy of the householder’s lease or mortgage agreement. (Click Here for Residency Forms A and B)  (NO PO Box numbers-only physical addresses)

  1. CUSTODY DOCUMENTS: Official court documents must be presented indicating which parent has primary physical custody of a child who does not reside with both biological parents.

  2. BIRTH CERTIFICATE AND SOCIAL SECURITY CARD

     

  3. VIRGINIA SCHOOL ENTRANCE HEALTH FORM (physical): this can usually be faxed from former Virginia school. (Fax:540-245-5189)

    • ***IF you are new to Virginia, you will also need to get a Virginia physical and have the doctor complete the Virginia School Entrance Health Form.

  4. HEALTH INFORMATION: A record of up-to-date immunizations may be faxed from the former school or from your physician's office. (Fax:540-245-5189) ALL rising 7th grade students MUST have the tetanus, diphtheria, pertussis (Tdap) booster shot prior to entry into the school and to also include vision or hearing evaluations and medication permission forms.  

     

  5. SIGN A REQUEST FOR STUDENT RECORDS:  You will need to sign a Records Release Form (Click here for records request form) with your student’s former school information so we may request the transfer of your student’s school records (grades, SOL scores, attendance, behavior, Special Education and 504 Plans).
